STARTALK Participant Surveys: Create a User Account

You must create an account before you can take the survey. After you create your account, you can re-use it every time you log in.

Please note that although you are required to provide your full name and birthdate before taking the survey, we will not be reporting any of your responses with any of this personally identifying information.

Important! Be sure to choose a username and password that you will remember. Your username and password are necessary for filling out the STARTALK surveys. If you attend any future STARTALK program, your username and password will still work for that programs' surveys as well.

	Your username must start with a letter (A-Z, uppercase or lowercase) or number (0-9), and may only contain letters, numbers, underscores, periods, and dashes.

	If you forget your password, you may have a new password sent to the email address you provide below.
