STARTALK Participant Surveys: Create a User Account

You must create an account before you can take the survey. After you create your account, you can re-use it every time you log in.

If you are the parent of one or more elementary students in STARTALK programs, please create separate accounts with different usernames for each child for whom you are completing the survey.

Username:	* required Your username must start with a letter (A-Z, uppercase or lowercase) or number (0-9), and may only contain letters, numbers, underscores, periods, and dashes.

Email address:	Your email address will not be published or shared without your consent. If you forget your password, you may have a new password sent to the email address you provide below. However, you are not required to provide an email address if you do not have one, or are not comfortable sharing your email address with us. Please use the same email address that you provided to your STARTALK summer program.

Important! Be sure to choose a username and password that you will remember. Your username and password are necessary for filling out the STARTALK surveys. If you attend any future STARTALK program, your username and password will still work for that programs' surveys as well.
